If you are sign up for the video filming competition (inclusive a compulsory attendence for the conference)

• Group Name
• Names of Members in the Group (Max 6 people)
• Contact Numbers of each member in the Group
• Email address of each members in the Group
• Institution of each members in the Group (ie, members may not come from the same institution)
